The data controller for this data is Karim Chadli T/A Advanced Hypnotherapy Services, and the data you have provided will be used and stored in the following ways depending on the data you provide to us. If the client is under 17 years of age, then the parent’s name and email address and/ or contact details will also be collected.

The information you provided on this New Client form as well as your medical and well-being data will be stored for 7 years (or if the client is a child then until his or her 25th birthday, or 26th birthday if the client was 17 when the treatment ended) in a paper-based format in a secure locked up storage.

Your name, address and email address data (and the parent’s if the client is under 17) will be stored for the purpose of performance of the contract.

Your well-being and medical data will be stored for the purpose of legitimate interest, in order to monitor progress of the agreed goals of the treatment, as well as in order to comply with legal obligations required by GDPR.

Your name, telephone number and email address (and the parent’s if the child is under 17 years of age) will be used to contact you with regards to your appointments.

If you (or your parent) opt in to receive our news, information and offers, your (or your parent’s if the client is under 17 years of age) name and email address will be stored in an electronic database and mailing system for the purpose of emailing occasional news and articles, and both database and the mailing system are password protected.

You (or your parent’s if the client is under 17 years of age) have the ability to unsubscribe from this at any time.

We will not pass your (or your parent’s if the client is under 17 years of age) details to any third party.

Your appointment dates and fees data will be stored in an electronic appointment database for 7 years to comply with legal obligations of tax calculation record keeping for HMRC.
